After a twenty- five year debate, the Netherlands became the word's first country to sanction active euthanasia if it is preceded by a very deliberate and prescribed collaboration of patient, family, and medical team. There must be a review by a panel consisting of a lawyer, professor of ethics and attending physician. This film profiles four people who are terminally ill at a palliative care facility in Rijnstadt Hospital, in Arnhem.

With patience and deliberation, Dr. Joep Douma probes the feelings and wishes of his patients and their families, assuring them that their decisions will be respected. They each are well aware that their illness is terminal. Henk Landa, attended lovingly by his devoted wife of many years, is unequivocal that he wishes to die with dignity, not struggling for breath as his cancer would inexorably dictate. His wife who was a nurse does not want to watch him endure the suffering she has seen in others.

Gerard whose cancer has spread to his liver has his fervent wish fulfilled, to be married to his homosexual partner of forty years. (Gay marriage has been legalized in Holland). In a touching bedside ceremony the two are wed. Gerard actually goes into remission and is able to return home to enjoy his remaining time with his new husband

With each of the four patients, their journey to choosing euthanasia, is carefully thought out. The doctor is a catalyst for exploring their feelings and values. Dying With Dignity profiles a remarkably humane medical system where the emotional and spiritual needs of patients are nourished by professionals who do not seem to be pressured by time constraints.
